Level Two is here!
If you completed a wellness screening and the HealthPath Questionnaire by April 30, now it's time to get going on Level Two of Healthy Purdue: the apples. Complete five of eight colored apples by November 7 and earn an additional $250 (before taxes) incentive.

The HealthPath Questionnaire (HPQ) is no longer available for Healthy Purdue 2008. If you have questions about your HPQ completion date, call StayWell's HelpLine at (800) 926-5455.
To verify that the $150 (before taxes) incentive has been credited, go to the OnePurdue Portal Login, log in using your career account and password, and view your salary statement (click Employee Self-Service and then payment). When viewing your pay stub, the incentive will be listed as "health plan inc. pymt."
