If you are an individual who is employed at Purdue University and are requesting reasonable accommodation(s) under the Americans with Disabilities Act of 1990 (ADA) and the ADA Amentments Act of 2008, you are encouraged to arrange a time to meet with Marcy Hintzman, Assistant Director for Compliance and Disability Services in the Office of Institutional Equity, or Pat D. Russell, ADA/Leaves/CDL Program Consultant in Human Resource Services. At this meeting, we ask that you bring with you:

• The completed Reasonable Accommodation Request Form
• The Authorization Form (please do not sign it until the meeting)
• Medical documentation that is pertinent to your condition and request for reasonable accommodation as indicated in the Health Care Evaluation Form

The Office of Institutional Equity or Human Resource Services will contact your health care provider as a part of the reasonable accommodation request process.
