Reasonable Accommodation Process and Forms
If you are an individual who is employed at Purdue University, believe you might be a person with a disability, and would like to request reasonable accommodation(s) under the Americans with Disabilities Act of 1990 (ADA) and the ADA Amendments Act of 2008, you are encouraged to arrange a time to meet with a Purdue Evaluator within the Office of the Vice President for Human Resources.You can reach a Purdue Evaluator at:

Please bring the following completed forms with you to your meeting with a Purdue Evaluator:
- The Reasonable Accommodation Request Form
- The Authorization Form (please do not sign it until the meeting)
- Medical documentation that is pertinent to your condition and request for reasonable accommodation as indicated in the Health Care Evaluation Form
The Office of the Vice President for Human Resources will contact your health care provider as a part of the reasonable accommodation request process.
