1. Describe your background and teaching experiences.  Include leadership activities, curriculum development, collaborations, in-service activities,  professional organizations, trips, seminars, conference attendance and/or presentations, experiences that have enhanced your teaching, topics that you would like to develop for your classroom, etc.

2. Explain what you hope to gain from the SISI program.

3. Submit a resume.

4. Submit a letter of recommendation/support from your school principal or administrator.  The letter should express support for your professional development and classroom implementation of standards-based inquiry activities. Your adminstrator must also agree to provide release time for you to attend the Hoosier Association of Science Teachers, Inc. (HASTI) conference in February 2005.
 
 

Application deadline is April 15, 2004; workshop is limited to 30 participants.
