CAROL BIRK, M.S., R.Ph.
Director, PharmaTAP, Purdue University
Executive Director, Indianapolis Patient Safety Coalition
Birk received her B.S. (Pharmacy) from Purdue University, M.S. (Pharmacy Administration) from University of Wisconsin, and completed a two year administrative residency at the University of Wisconsin. Areas of focus include: pharmacy program management and development, contemporary strategies to improve the safety and efficiency of the medication use process, medication use accreditation and regulatory standards, leadership, mentoring and organizational design, and performance improvement and measurement. She has extensive experience in hospital pharmacy practice and healthcare performance improvement.
In June 2008, Birk was named as the Indianapolis Patient Safety Coalition’s new Executive Director. Since being founded in 2003, the Coalition provides a forum for Indianapolis-area hospitals to share information about ‘best practices’ and work together to solve the most concerning patient safety issues in Indianapolis hospitals in a non-punitive setting.

Knotts has 20 years experience in performance improvement; most recently as Executive Vice President for Corporate Compliance at Cummins Behavioral Healthcare Systems, Inc. In her role, Theresa had the privilege of playing a leadership role in an organization receiving national recognition by being the winner of the 2007 Negley Award for Excellence in Risk Management. She also has extensive experience in risk management, patient safety, Joint Commission accreditation for hospitals and behavioral health. She has been a Certified Professional in Healthcare Quality (CPHQ) since 1991. As a skilled nurse she worked in medical and cardiac intensive care as well as the medical floor. She earned her B.S.N. at Indiana University.

Hultgren's experience includes his work in pharmacy practice at Indiana University Hospital (IU). While at IU, he participated or led committees to develop and implement solutions to medication related issues.
