Regenstrief National Center for Medical Device Informatics

Realizing that alarm safety was an emerging concern among hospital administrators and national regulatory organizations, RCHE focused attention on this escalating patient safety challenge in 2010 before it reached the level of high national awareness. Using Purdue HUBzero® technology (developed through a $38M investment from the National Science Foundation that has been leveraged to create more than 40 HUBs that support a wide spectrum of scientific communities), RCHE established a HIPAA-aligned HUB ( as a portal to support an internet accessible, national “Drug Limit Alert” database.  Initially called Infusion Pump Informatics (IPI), this system is now named the Regenstrief National Center for Medical Device Informatics (REMEDI) Central.  Hospital users submit the following types of infusion pump data to REMEDI Central: 1) alert data generated when the clinician programs the pump outside the limits of the Dose Error Reduction Software (DERS); 2) the compliance data indicating how often clinicians use the smart pump features and 3) their hospitals’ drug limit libraries.

As of April 2016, 149 hospitals across the nation deposit data from smart pumps delivering IV drugs to patients into the REMEDI Central functionality of CatalyzeCare and generate comparative reports summarizing their hospital’s data relative to other hospitals in the database. Most importantly, the reports are used in real time to identify candidate drugs for in-house changes to the “drug limit libraries” that hospital pharmacists generate to program smart pumps delivering drugs. This resource has grown dramatically since its inception and during the last four months alone growth has averaged one new hospital per week. RCHE sponsors three national conferences a year, one in-person and two “virtual,” for this “Community of Practice”. The in-person conference is attended by pharmacists, nurses, patient and medication safety officers, and representatives of various national organizations who have an interest in infusion pump safety. Given the Joint Commission’s edict on alarm management, it is not surprising that the Association for the Advancement of Medical Instrumentation (AAMI), the American Society of Health-System Pharmacists (ASHP), and the Institute for Safe Medication Practices (ISMP) all have contacted RCHE to explore: 1) significantly expanding REMEDI Central’s capabilities, and 2) leading the effort to establish a national standard for infusion medication limits.

RCHE is exponentially expanding to enable an even greater impact on the safety and quality of healthcare delivery nation-wide. We are actively working on projects to: 1) significantly increase the capacity of the current system to allow more users and improved performance; 2) capture all infusion pump alarms, not just those generated against the drug limit libraries; 3) dramatically increase the capabilities to include physiological sensing monitor data; and most important 4) linking smart pump and physiological sensing monitor data to patient data to better measure impact. Forming new communities around these additional medical devices will provide fertile ground for Purdue researchers and our partners to examine not only the use of these devices, but how these communities advance patient safety and quality.
