Don’t forget about community, training, and medical alternatives

Community alternative

If you take part in health-related offerings from other campus programs, hospitals, fitness centers, or community organizations, you may receive credit for a purple or red apple, depending on the specifics of the offering. Before taking part, contact your campus program to review the details and receive approval for the alternative. Submit a completed Community Alternative Form and evidence of participation in the event to your campus program to earn an apple.

Training alternative

If you train for and complete a competitive exercise event such as a 10K, marathon, or triathlon in 2009, you can get credit for one purple apple. Print a Training Alternative Form. Submit the completed form and evidence of your participation in the event to your campus program to earn an apple.

Medical alternative

If you are unable to earn the apples due to a medical condition, contact your campus program to develop an alternative way to qualify for the 2010 medical plan premium reduction.


1Get a wellness screening

To achieve good health, it’s important that we know where we are starting from. That’s the purpose of the wellness screening. A wellness screening is a chance for you and your spouse/same-sex domestic partner to learn more about your current health status and to identify any areas for improvement. The wellness screening measures your:

Height

Weight
Blood pressure
Total cholesterol

HDL
Glucose

If you haven’t already, be sure to schedule a screening through your campus program, or make an appointment with your health care provider to get these important health measures.


2 Take the HPQ and get your apples

In 2009, you no longer have to complete the HPQ before earning your apples – you can complete these tasks in any order you choose. However, don’t forget that you need to have a wellness screening before completing the HPQ.


HPQ... your personal health report




Within six months of completing your wellness screening, you need to complete the online HPQ. This confidential tool asks you a series of health-related questions. After you complete the assessment, the information you provided is used to create your confidential, personal report.

Log in using the user ID you created last year. If this is your first time logging in, click on the “register now” link.
Complete your online HPQ and fill in your screening values (blood pressure, total cholesterol, HDL, and glucose).


To request a paper HPQ, call the StayWell HelpLine at 800-926-5455, Monday – Thursday from 9 a.m. to 9 p.m. ET, Fridays from 9 a.m. to 7 p.m. ET, or Saturdays from 9 a.m. to 1 p.m. ET.

 

It’s time to get your apples
All you need is four of the following six apples, plus the wellness screening and HPQ, and then the $200 medical plan premium reduction in 2010 is yours!

 

Blue Apple StayWell Health Coaching

StayWell Health Management provides health coaching through the “Next Steps” program. Coaching is available after you complete the HPQ. Select a topic based on your questionnaire results. Coaching can be done by phone or by mail.

  • Phone coaching – Phone coaching takes about three to five months. You’ll need to participate in a minimum of three calls (you may request additional calls) and complete an evaluation.
  • Mail-based coaching – Mail-based coaching takes about six months. You’ll receive a minimum of four monthly mailings and need to complete an evaluation.

So be sure to carefully review your HPQ results and, if needed, register for the “Next Steps” program by June 30 to ensure you have enough time to complete the coaching and evaluation by the November 20 deadline.

Blue Apple StayWell Online Healthy Living Programs

Available on StayWell’s Web site, this six-module interactive program offers weekly online activities. Choose one health topic and complete the six modules and an assessment to earn a white apple.


We recommend that you register by September 11 to have enough time to complete the program by the November 20 deadline.

Blue Apple Behavior and Lifestyle Change Programs

Programs help you develop skills and make behavior or lifestyle changes. They meet regularly over six to 14 weeks or for a minimum of six hours. In 2009, you may earn one purple apple toward your four. There are three ways to complete a program:

  • Campus offerings (in person or online/e-mail based)
  • Community alternative
  • Training alternative

Blue Apple Health Awareness

This category is for health topic presentations of about 45-60 minutes. You may earn one red apple toward your four for the year. There are four ways to earn a red apple.

  • Campus offerings
  • Purdue’s online offerings
  • StayWell online education centers (complete three for one red apple)
  • Community alternative

Blue Apple Health Care/Counseling/Preventive Screenings

Completion of this apple, which is self-tracked, involves either:

  • Meeting with your health care provider or counselor to discuss one aspect of your HPQ results, such as mental well-being, stress, weight, cholesterol, blood pressure, or tobacco use, or
  • Completing all recommended screenings for your age and gender, as suggested in your HPQ results.

Blue Apple Self-Report

This self-tracked apple is earned by doing one of these activities for six months in 2009:

  • Avoiding tobacco products
  • Eating five fruits and vegetables most days of the week
  • Exercising for 30 minutes most days of the week
  • Doing weekly relaxation activities
    (for example, yoga, tai chi, deep breathing)

3 Track your progress

Campus program staff and StayWell track your participation and award apples. Checking your progresspdf is just a mouse click away.
The "Check My Apples” link at the top of any Healthy Purdue page will take you to the WorkLife Programs Online Registration System.

 

For the two self-tracked apples (yellow and orange), you are responsible for logging into the WorkLife registration system, accessing the tracking, and clicking the “Confirm” button for the apples you have completed.
