Initial Contact With CAPS

Students who have never utilized our services in the past or are returning after an absence from services can schedule an initial assessment (IA) online at https://myhealth.push.purdue.edu. Our assessment specialists will meet with you for an initial consultation to assess your situation, clarify presenting concerns, and facilitate the matching of services based on your individual need and the variety of resources and types of treatment that we have available. At the end of the IA, you will have a recommended plan and clear next steps to address your concerns.

What to expect:

Initial assessments (IA) are ONE HOUR; half of the time will be spent completing paperwork and the other half with a specialist. Our assessment specialists will meet with you briefly for an initial consultation to assess your situation, clarify presenting concerns, and facilitate the matching of services based on your individual need and the variety of resources and types of treatment that we have available. At the end of the IA, you will have a recommended plan and clear next steps to address your concerns. 

When to arrive:

Please arrive at your scheduled appointment time.  No need to arrive early for paperwork, as that is already built into the hour you are here for your initial assessment. 

Where to go:

Initial assessments take place on the 2nd floor of PUSH. Check in half way down the hallway in Room 224 or 228.

New Client Information Forms

All initial forms for CAPS are completed in our waiting room on our computer as part of your Initial Assessment (IA).

There are four items you will complete.
  • Contact information (name, date of birth, PUID, phone, address)
  • Client Information form (demographic information and what brings you to CAPS)
  • CCAPS-62 Assessment measure based on the last two weeks
  • Current semester schedule
Completing these forms takes approximately 15-30 minutes of your one hour Initial Assessment appointment and provides helpful information so we can best serve you.

Emergency/Crisis Contact

Call 911 if you are experiencing a Mental Health Emergency.

  • Ask to speak to the Purdue Police CIT Officer.

Come into CAPS or call us at (765) 494-6995 for mental health crisis situations. A therapist will conduct a crisis assessment and plan for next steps for you.

  • Monday - Friday: 8:00 a.m. - 5:00 p.m.
  • After business hours, call 765-494-6995, press 1, and you will speak with a clinician who will assess your situation and help you.

Alcohol and Other Drug (AOD) Program

  • AOD Individual Therapy or Consultation
  • University- or Court-Mandated AOD Evaluation
    • For mandated AOD evaluations you must bring your referral documents (from court or the University)
