Form SS-5:Application for a Social Security Number	Complete and take to Social Security Administration
Permission to Request Social Security Number	This letter should be provided by the Business Office to the Employee to complete and send to the Tax Department along with their Glacier documentation. This letter gives the SSA approval from the employee to fax the new social security number to Purdue University Tax Department.
Request for Prior Year Social Security or Medicare Tax Refund:	This letter must be provided to Tax Department by an employee requesting a refund of social security tax from a prior year.
