Duplicate or Corrected W-2, 1042S, and 1099-MISC

Employees should choose one of the methods below to request a duplicate or correct tax form:

  1. Fax (765-494-1728) a completed and signed Duplicate Tax Request Form to the Tax Group.
  2. Mail a completed and signed Duplicate Tax Request Form to the Tax Group.

  3. Contact the business manager or payroll clerk for your home department.  The department will fill out and sign the request form on your behalf and forward it to the Tax Group.
    *Due to data security issues, please DO NOT email the request form.

    For pickup requests, duplicate forms will be available at the front desk of Freehafer Hall (401 S. Grant Street).  Photo identification is required to pick up forms.

Tax Office business hours are Monday - Friday 8:00 am - 5:00 pm
