Submit Leave Action Form for All Military Leaves - 04/12/12
Human Resources Service Center reminds business offices to submit a Leave Action Form (LAF) for any Purdue employee who is out for paid or unpaid military leave.
The University grants up to 15 work days of paid military leave per calendar year to allow employees to fulfill their responsibility when ordered to report for military duty. An LAF is needed to ensure the 15 days of paid leave are administered properly.
The University also grants unpaid leave for military duty that extends beyond 15 work days in a calendar year. LAFs also are needed to properly record and report the unpaid military leave.
The LAF, along with a copy of the military orders for each military leave event — paid or unpaid — should be sent to the HR Service Center. Through the Leave Reports, HR is able track employees who have returned from a leave, as well as those who are currently serving. It also assures that University policy is being applied properly.
