Leave Action Form Revised - 10/14/08

The Leave Action Form has been revised to assist in its processing and to ensure essential information is provided. Key changes include:

   -- Area to specify new or revised form
   -- Section for org unit name, org unit number, and pay area
   -- Added line in “Return to Work” section: Employee is returning from what type of leave?
   -- Check box for Paid Parental Leave added in the comments section of the form
   -- Routing instructions added to the bottom of the form

The new Leave Action Form can be viewed online by visiting the Human Resources Web site and selecting “Forms” under the “Resources” gold tab.
