Veterans Status Revised on Self-Identification Compliance Form - 04/9/09

The Self-Identification Compliance Form (SICF) has been updated to more clearly define the discharge date for active military service.

The revision on Page 2 of the form reads:

If you are a veteran who served on active duty in the U.S. military, ground, naval or air service and have been discharged or released, please indicate your most recent discharge date (mm/dd/yyyy) as specified on your most recent DD214: ____/___/____.

Please note the change and update Self-Identification Compliance Form or saved files to its April 2009 version. Staff members can update their veterans status information by either completing the SICF or using the Employee Self Service (ESS) portal, available through the OnePurdue Portal which can be accessed through the Purdue home page under “Access Purdue.”

The Office of Institutional Equity, formerly the Affirmative Action Office, will be sending out information to all employees via email on Monday, April 13 to resurvey the workforce before academic year faculty and staff depart at the end of this semester. This effort will be communicated in Purdue Today and Inside Purdue to advise veterans to update their information.
