Signature Authorization

 

Effective Date: Immediately

Effective immediately the University Chief Accountant for the West Lafayette Campus and the Chief Business Officers for the Regional Campuses are authorized to approve signature authorization requests for the signing of payroll vouchers, requisitions, check requests, and other business documents by other than the head of the department or office.

Signature authorization approvals are obtained by submitting Business Office Form 0009 for the West Lafayette Campus and 0009-A for the Regional Campuses. This form must be signed personally by the head of the department or office and include the signatures of staff members designated to sign for the head.

The number of persons authorized to sign payroll certifications, Purchasing requisitions (Form 12), and check requests (Form 56) for the head of the department or office should be limited. For most departments, three (3) persons so authorized should be adequate. Additional authorizations to sign internal business documents (e.g. Stores requisitions, journal vouchers, Duplicating requests) may be made. Whether the authorized person will sign only their own name or the department head's name followed by their own name is the decision of the department head. When revisions of signature authorizations are necessary, a new form must be submitted with the signatures of all individuals authorized to sign for the head. When a new head of a campus, school, division, department, or office is appointed, it will be necessary for the head to file a newly-completed Form 0009 or 0009-A.

The delegation of signing business documents does not relieve the delegating official of the final responsibility for the accuracy of the statements, information, or data contained in the forms.

In order to bring records up-to-date under the new format each area is asked to submit the revised Form 0009 or 0009-A. Additional copies can be secured by calling 749-6184.
