Office of the Registrar Forms:
Form 23 , Course Request, used to change undergraduate students' course registration.
Form 27 , Division/Section Change.
Form 70 , Audit Permission.
Form 350, Academic Record Change, used for academic record changes, such as:
- Exemption from COM 114, which is permitted by some schools and departments.
- Classification change, often necessary if a student has dropped or not passed some coursework.
- Adjustment of graduation index. Necessary, for example, when transfer credit is substituted for a course or when a student completes an equivalent course at the University or a regional campus that does not carry the identical number to the course being replaced.
Form 380 , Request to Have Degree Awarded at Another Campus. Must be submitted to the registrar of the home campus before the seventh week of the semester or fourth week of summer session.
Form 385 , Request to Pursue Multiple Degree Programs Concurrently. Must be approved before the second week of the session in which the degrees are to be conferred.
Form 390, Report of Directed Credit .

Office of Admissions Forms:
CTR Form 5, Transfer Credit Evaluation Report, request for transfer credit from another accredited college or university. Student must make the request before enrolling in the course.
Form 231, Purdue University Credit/Placement Exam Authorization. Must be signed by an advisor before a student can take a credit/placement exam.
