| Information Label
The Office of Admissions sends an information label for each admitted student to the advising office of the student's school or program. Included with the information label are four home address labels for school/program mailings.
Please see explanations below this example label:

Line 1: Name, school.
Line 2: Social Security number, sex, date of birth, residence code, status (beginner, transfer, RC-Trans).
Line 3: Semester of entry, action (see below), date admitted, class, major, high school code, high school graduation date.
The " action " or admission status that appears on the information label (electronically or on hard copy) will be indicated with one of these codes:
A (ADM) — Admitted
B (PRO) — Admitted on probation
D (DEN) — Denied admission
I (INC) — Incomplete application
E (TSE) — Evaluation
N (NFP) — Cancelled because $30 application fee not paid
C — Student cancelled the admission or application
3 (CAN) — Cancelled because $100 deposit not paid after admission
Line 4: SAT-Verbal, SAT-Math, SAT composite, high school rank, high school class size, probability of earning 2.0 in first semester at Purdue, home phone.
Line 5: ACT scores in English, math, reading, and scientific reasoning, ACT composite.
Line 6: Purdue-calculated achievement score composites in English, math 1, math 2, chemistry, physics, biology; foreign language taken in high school, number of semesters completed, and grade average.
Line 7: Overall high school grade average and high school courses listed by number of semesters and grade average: English, math, science, language, speech.
