Identity Verification for Password Changes
In accordance with requirements of the Health Insurance Portability and Accountability Act (HIPAA) and University security guidelines, it is the policy of Student Services Workstation Technology to verify the identity of any individual who is requesting a password change to supported systems. If the individual is unable to accurately provide the required information, our staff will be unable to change the password at that time and the individual will be directed to provide, in person, a valid photo ID to a support staff member in Schleman B37 or utilize the Career Account swipe station in Stewart G-65

The procedure is as follows:
- The only person who can request a password change is the user themselves.
- The individual must supply all of the following information to support staff:
- Full name (first name, middle initial, last name) and their username
- PUID (located on your staff ID card)
- Date of birth
- Local mailing address
If any information provided by the individual is incorrect, the customer must:
- Bring a valid photo ID to the SSWT department located in Schleman B-37 for identity validation - - OR - -
- Swipe their Purdue ID card at a the Career Account card swipe station located in STEW G-65
As a reminder, SSWT (44357) can only assist with changing your career account (network login) password. For Mainframe password assistance, you must call the ITaP Customer Service Center at 44000. You will be required to supply Identity Verification information to them as well. |
