Protocol [IBC Form 2A]
A Triennial
Surveillance for Biohazardous
Agents and rDNA Research
[IBC Form 3A] has been
developed to aid the Office
of Research Administration
in monitoring active protocols
that have been approved
by the IBC. The form will
be sent by the IBC office
to all principal investigators
prior to the three-year
anniversary of their most
recent approval date. Completion
and resubmission of this
form - prior to the expiration
date of the current approved
protocol - will be utilized
to renew the approval of
the protocol for an additional
three years providing there
are no questions or concerns
when the surveillance form
is reviewed by the IBC
Chair.
