FORM 101W

*IMPORTANT: Please read Below*

Purdue University staff members needing access to student data through the Office of the Registrar's WebServ application for any of the following reasons must receive authorization from the Dean or Director of their respective college/school:

Retrieving student educational program information
Informing students about co-curricular opportunities
Performing academic record keeping

This approval must be sought through the completion of Office of the Registrar Form 101W.

Prior to seeking this authorization, staff members must understand the provisions of the Family Educational Rights and Privacy Act of 1974. Staff members may verify their understanding of this legislation by completing the training found at the following Web site:

Completed Form 101Ws should be sent to Michael Reynolds, Office of the Registrar, HOVD.
