Guidelines for Classroom Examination for Qualified Students

The Testing Center is a resource for instructors who are unable to provide testing accommodations within their departments for eligible students with disabilities. A wide range of accommodations is provided at the Testing Center for students whose eligibility has been certified by Adaptive Programs. Some of the accommodations include extended time, a distraction-reduced environment, use of a computer and/or adaptive technology, and a private room. ODOS policies and procedures ensure the integrity and security of classroom exams proctored by the Center.

  1. Instructor Guidelines for Testing Center Use:
    Instructors or their designees are responsible for submitting materials to the Testing Center either in person or electronically (odostc@purdue.edu). The exam and Classroom Exam Form must be received in the Testing Center twenty-four hours prior to the student's appointment time. A completed form must be attached to every exam. Questions should be directed to the Testing Center staff at 4-1146 or at odostc@purdue.edu.

    If the student needs an exam in Braille, the exam and form must be received in the Testing Center four working days prior to the appointment date. A completed Classroom Exam Form must be attached to every exam.

    PLEASE NOTE:

    Students must confer with instructors regarding an approved date and time of exams. Students are then responsible for calling the Testing Center to schedule an appointment. Because space is limited, students are scheduled on a first come-first served basis.

    The ODOS Testing Center schedules student appointments based on the time designated in the individual student's accommodation letter issued by Adaptive Programs. For this reason we are unable to proctor exams that allow extended time above & beyond the student's approved accommodations. Therefore, instructors who wish to extend accommodations above & beyond the approved accommodations will need to test the student within their department.

    The ODOS Testing Center will only allow students 1reschedule” opportunity per exam. However, a “reschedule” will be deemed necessary by the ODOS Testing Center for any of the following reasons:

    1.  A student not showing up for a scheduled appointment for any reason.
    2.  A student making an appointment change within 24 hours prior to the original              appointment.
    3.  The late arrival of a student beyond a 15 minute grace period.

    Please note that exams must be scheduled for the full allotment of time designated in the student accommodation letters. Additionally, if a reschedule is required by the Testing Center, it is at the descretion of the instructor to grant permission for a 2nd appointment so that the exam may be completed.

    The instructor will be responsible for administering exams if additional requests or agreements are made between the student & instructor beyond these Testing Center guidelines and the designated accommodations of Adaptive Programs.

    Instructors or their designees may pick up completed exams or have them returned via campus mail; however, the Testing Center is not responsible for damage or loss if exams are returned via campus mail. Students are not permitted to transport exams to or from the Testing Center.                       

  2. Student Instructions to Schedule Exams:

    Contact the Testing Center at 49-41146. The Testing Center is located in Schleman Hall, Room B42 and is open Monday through Friday from 8 a.m. to 5 p.m.

    Schedule at least one week in advance of regular exam date. Because space is limited, schedule midterm and final exams as early in the semester as possible.

Contact your instructor if you need to reschedule an exam. The Testing Center staff will not reschedule an appointment unless this contact is made.

Students will only be allowed 1 reschedule appointment per examination.

The ODOS Testing Center will require an exam be rescheduled for any of the following reasons:

1. Not showing up for a scheduled appointment for any reason.
2. An appointment change within 24 hours prior to the original appointment.
3. Late arrival of the student beyond a 15 minute grace period. Within the 15 minute grace period, the amount of time that the student is late will be deducted from the total time scheduled for the exam. Students arriving beyond 15 minutes of the appointment time will be required to reschedule their exams.

Please note that exams must be scheduled for the full allotment of time set forth in the student's accommodation letter. Please note if a reschedule is required by the Testing Center, the instructor must additionally grant permission for a 2nd appointment so that the exam may be completed.

Be on time for your appointment. If you arrive late for an exam, you will forfeit the amount of time you are late.

Bring your student identification card. You will not be allowed to take the exam without showing your student ID.

The Testing Center will no longer allow outerwear of any kind to be worn in the testing rooms. This includes the following:

- Hooded sweatshirts with pockets (hoodies)
- Coats
- Jackets
- Vests with pockets
- Other items deemed inappropriate by testing staff

Please dress accordingly!

Standards of student conduct and academic integrity apply in the Testing Center.
