 1.  GS Form 9: Thesis Acceptance Form

Bring this completed form to your deposit appointment.

We require an original, paper copy, signed by your committee, major professor, head of departmental graduate program, and (if applicable) departmental thesis format advisor. Major professors need to sign in multiple places. Please the check form carefully.

If the thesis/dissertation is designated as Confidential by this form, GS Form 15 is required. See Special Forms section below.

 2.  Graduate School Form 32: Thesis/Dissertation Agreement, Publication Delay, and Research Integrity & Copyright Disclaimer Form

Bring this completed form to your deposit appointment.

As of September 1, 2014, iThenticate plagiarism screening by your major professor must take place prior to completion of GS Form 32. Satisfaction of this requirement will be certified by both major professor and degree candidate signing in Section III.

3.  Graduate School Exit Questionnaire: Certificate of Completion

Bring this certificate to your deposit appointment.

Print certificate of completion when you finish. You will not be able to log in again.

Regional campus students do not complete the Graduate School Exit Questionnaire.

4.  GS Form 30: Cover Page for Electronic Thesis Deposit 

(Formerly called ETD Form 9)

Attach this form in PDF form before the first page of your thesis/dissertation.

Type the names of your committee members and the head of your departmental graduate program as they appear on your signed GS Form 9. No signatures should appear on this form.

GS Form 36, Graduate School Thesis Title Abbreviation Form

If you are using this form, bring it to your deposit appointment.

Required only when a thesis/dissertation title is over 200 characters in length (including spaces and punctuation).  

 

GS Form 15, Request for Confidentiality

If you are using this form, bring it to your deposit appointment

Confidentiality and Delay of Publication, while different programs, can be utilized individually or in tandem depending on author needs. 

 

CONFIDENTIALITY

Purdue University program requested via GS Form 15 (Request for Confidentiality)

  • thesis remains at the Graduate School and is not transmitted to ProQuest Information & Learning until released (normally after 1 to 2 years)

  • thesis is not available for viewing and is not available through ProQuest or Purdue Libraries catalogs during confidentiality period

  • this program is commonly used when applying for patents, for pending publications, or when proprietary rights are involved

 

DELAY OF PUBLICATION

ProQuest Information & Learning program requested during ETD submission with major professor’s concurrence on GS Form 32 (Thesis/Dissertation Agreement, Publication Delay, and Copyright Disclaimer)thesis is transmitted to ProQuest for publication after normal Graduate School processing—ProQuest imposes the requested delay period upon receipt

  • delays imposed are normally 6 months to 2 years, but can be up to indefinite depending on circumstances

  • after ProQuest processing, the thesis abstract and citation information are made available for viewing online, but the thesis itself is not available for viewing during the delay period

  • this program is commonly used when applying for patents, for pending publications, or when proprietary rights are involved
