Compliance with Public Health Code of Indiana
Executive Memorandum No. A-45 issued by President Hovde on 5 September 1950 called attention to the requirement of the Public Health Code of Indiana. The law requires that all employees of Purdue University have a chest X-ray examination for tuberculosis every three years. The University has made provision for such X-ray examinations without cost to the employees by the establishment of an X-ray Services Department in the School of Pharmacy. Employees may comply with the law by:
- Having an X-ray examination made by the X-ray Services Department in the School of Pharmacy. Appointment forms must be secured from the Student Health Service.
- Having an X-ray examination made by the X-ray units provided by the State Board of Health.
- Having an X-ray examination made by a physician at the expense of the employee. A certificate of such examination, if made within the last 3 years, must be filed with the University.
If your last chest X-ray was made three years or more ago, or if you are a new employee and have not had a chest X-ray made in the last three years, please make the necessary arrangements to be examined at once. After your examination, complete the certification below and give it to the head of your department.
